Brief Description Organization

Provides support to older adults who want to continue living independently in their own homes. The following 3 programs are intended to support older adults who want to remain independent in their own homes and include:
  • Bridges Day Program
  • Geriatric Community Rehabilitation
  • Adult Day Program

Who is eligible to use your service?

"Eligibility For seniors from Lethbridge and area who: Want to continue living in their homes, but have some risks Need 3 or more services from the program Are able to come to the program and other services Are able to move without a mechanical lift Are medically stable Referral Needed A written referral from your family doctor, home care RN or geriatrician to the Senior's Day Program is needed. Service Access Healthcare providers should consult the Alberta Referral Directory for service referral information. Once the referral is accepted, seniors are assessed to determine which of the 3 programs meets their needs."
