The Transitional Aged Youth (TAY) Case Management Program provides community-based support for young people living with mental illness and addictions challenges, including those transitioning between the youth and adult mental health systems.
The program serves youth aged 15-26 who live in the west end of Toronto (west of Yonge St. to Kipling Ave., south of Eglinton to the Lake) and are living with mental health and addictions issues.
