A not-for-profit health service organization in the west end of Toronto. Services include supports for seniors, caregivers, and people living with mental health and addictions concerns. The Transitional Aged Youth (TAY) Case Management Program provides community-based support for young people living with mental illness and addictions challenges, including those transitioning between the youth and adult mental health systems. The program serves youth aged 15-26 who live in the west end of Toronto (west of Yonge St. to Kipling Ave., south of Eglinton to the Lake) and are living with mental health and addictions issues.
