Claresholm & District FCSS (Family and Community Support Services) provides local preventative community driven social programs that encourage volunteer participation. Claresholm Addiction and Mental Health Clinic – Accessible mental health services including mental health assessment, treatment, information, referral and follow-up for individuals of all ages and their families. Claresholm & District Transportation Society – Provides transportation for those who are unable to find transportation to medical appointments outside of Claresholm. Claresholm Centre for Mental Health & Addictions (Alberta Mental Health) – A community focused psychiatric facility providing treatment, educational, rehabilitation, and supportive services. Services are dedicated to maximizing the ability of mental health consumers to live independently in the environment of their choice. Willow Creek Continuing Care Center – Provides long-term residential care, including nursing and personal care, physical therapy, occupational therapy, recreational therapy and pharmacy services. Claresholm Community Home Care Assists clients to achieve optimal health and well-being by providing services to support personal independence in their homes. Provides services such as the Medication Assistance Program, foot care, blood pressure monitoring, and other services.
