Home care provider in the Halton region (Burlington, Oakville, Halton Hills, Milton). Registered charity.
Emotional, social and spiritual support for those with life-limiting illnesses and their families.
Hospice palliative care is all about supporting people living with an advanced illness. The goal is to improve quality of life for the person with the illness and their family members.
Our Hospice Care program supports your emotional, psychological, spiritual, practical and social needs. While you may have many health care providers involved in caring for your physical health (including potentially Acclaim Health Nurses or Personal Support Workers) this program is focused on what you may be feeling and thinking about.
This program is offered free of charge through the generosity of our volunteers, funders and donors.
We’re Here to Listen
If you need to talk, we’re here to listen. Or we can simply provide a comforting presence.
With deep compassion and without judgement, this is your safe space to talk about the good, the bad, and the difficulties of your personal situation.
Our team of caring staff and highly skilled volunteers will walk with you on your journey – every step of the way. We will listen, work hard to understand your needs, support you and your family, share resources, and connect you to additional community services as your needs change.
Depending on your wishes and ability, volunteers may be able to take you out for an activity or to socialize.
Carer Café – Caregiver Support Group
Are you a family member or friend caring for a loved one with Cancer, Parkinson’s, advanced CHF, COPD, or other life-limiting physical condition?
Then you are a “Carer” – a critical partner in health care and an individual with your own needs.
The Carer Cafe is a chance to come together with other carers and share your experiences with others who are navigating the same healthcare system, needing similar information and feeling the same frustrations, joys and grief.
