To join a Club, you must complete a referral either through the Central Registry or Home and Community Care Support Services (formerly the Local Health Integration Network, LHIN) based on your place of residence in Halton. Intended for adults in Burlington who have a moderate or advanced dementia and require frequent cuing, direction, and supervision. Requirements: 18 years or older, have a valid health card number, NOT live in a long term health care setting Criteria: be able to tolerate a group setting, be medically stable, not pose a risk to self or others, and be able to safely perform a one-person assisted transfer (Please note a few programs accommodate heavier transfer needs - consult further with programs). Cannot accommodate: Clients who require tube feeding or mechanical lift transfers
