Join The Integrated Care Pathway!
One integrated pathway where care partners, families, and neighbors all use a single app from hospital to home, community, and senior care.
Families and seniors can seamlessly connect to the healthcare and community organizations they need, exactly when they need them.
Three co-designed programs:
Together Going Home
This program supports individuals and their care partners from hospital-to-home through a care partner needs assessment, follow-up, and service connections designed to promote recovery and reduce hospital readmissions.

Together We Care
A rural Alberta, community-led initiative that co-designed Team CarePal to connect families and neighbors with local volunteers and services—helping reduce caregiver burnout and isolation.

Together In Care
A program that helps senior care homes communicate and build stronger relationships with families/care partners, reduce staff workload, and improve resident experience. Co-designed by families, staff and lived experiences.

Organization Benefits
Your organization will get our platform that comes with a link that offers a branded app with your logo to all your families.
You will be showcased in our app for other family caregivers to find and subscribe their app to your platform.
You have worked hard to create education and courses for your clients. Set it up in the platform, and caregivers will have access to it in their app.
Switching between multiple communities in the app is simple—just a push of a button. Your organization can host a private volunteer hub, where families connect in a safe space and local volunteers are matched directly to real community needs.
Our platform enables better care coordination among patients, family caregivers, and healthcare providers. By using our product, your organization can streamline communication and collaboration, reduce errors and delays in care, and improve overall health outcomes for patients.
Supporting caregivers effectively leads to decreased hospitalizations, reduced incidence of Coronary Heart Disease, fewer mental health challenges, lowered stress levels, diminished frequency of doctor visits, and a decrease in Long Term Care admissions. (Team CarePal Health Economic Model results)
Your organization can save time and resources by automating routine tasks, such as scheduling appointments, onboarding, and updating families. By reducing administrative burden, you can focus on delivering high-quality, caregiver-centered care.
Data-based decisions enhances the care quality through personalized treatments, optimizes resource allocation, enables predictive interventions for better outcomes, reduces caregiver burnout by monitoring workloads and feedback, and identifies training needs for skill enhancement.
